Medieval · c1250–1500
Health in the medieval town
Most medieval people lived in the countryside, but towns grew crowded and dirty. With no clean water supply and no sewers, health depended on how well a town coped with its own waste.
Water came from wells, rivers and conduits — often the same water people threw waste into.
Butchers, tanners and dyers dumped blood, guts and chemicals into streets and rivers.
Human waste went into cesspits and middens; "gong farmers" emptied cesspits by hand at night.
Who was responsible? Town councils passed rules against dumping waste and fouling water, and appointed men to keep streets clean — but there was no national government action on health.

Monasteries and the Black Death
Monasteries were often the healthiest places: monks planned clean water supplies, drains, latrines and infirmaries — because they were wealthy and valued cleanliness for religious reasons.
In 1348–49 the Black Death (plague) killed perhaps a third to a half of England's population. People did not understand its cause and tried a mix of responses:
Religious explanations — a punishment from God; some prayed or joined processions of flagellants.
Miasma — the idea that "bad air" caused disease, so people burned herbs and cleaned streets.
Practical steps — some towns tried to isolate the sick and clear rubbish.
Misconception alert: medieval people had no idea about germs (bacteria were not seen until the 1600s and not linked to disease until the 1800s). Their responses were based on religion and the "bad air" theory.

Early Modern · c1500–1750
Growing towns, same problems
Towns — above all London — grew much larger. Living conditions stayed poor, and outbreaks of plague kept returning.
The Great Plague of 1665 killed roughly a fifth of London's population. Towns tried quarantine — shutting up infected houses (marked with a red cross) and killing cats and dogs.
Pest houses (isolation houses) were built to keep plague victims away from others.
People still believed in miasma; there was still no understanding of germs.
Continuity: across the Medieval and Early Modern periods, the key ideas about disease (God, miasma) and the lack of national government action stayed broadly the same. Change was slow.
Early Modern · small changes
Some progress by 1750
There were a few improvements before the Industrial Revolution:
Improved water supply in London, e.g. the New River (opened 1613) brought fresh water into the city.
The Great Fire of London (1666) cleared slums; rebuilding used more brick and stone and wider streets.
Growing trade brought new foods and, later, tea and coffee (boiled water, safer to drink).
These were local and accidental improvements, not a planned public-health system. Governments still believed health was a personal or local matter, not their job — a policy later called "laissez-faire" ("leave alone").

The crisis of the industrial town
The Industrial Revolution pulled huge numbers of people into fast-growing cities like Manchester, Leeds and Birmingham. Housing could not keep up, and conditions became a public-health disaster.
Cheap "back-to-back" houses shared a single privy and water pump — sewage often seeped into the drinking water.
Overcrowding: cheap "back-to-back" houses with no gardens, shared toilets and no clean water.
Cholera epidemics (1831, 1848, 1854, 1866) killed tens of thousands — spread through sewage-contaminated water.
Industrial · the turning point
Chadwick and Snow
Two men changed how people understood and tackled the crisis:
Edwin Chadwick's Report on the Sanitary Condition of the Labouring Population (1842) proved that dirt, bad drainage and poor housing caused disease and cost the country money. He argued for clean water and proper sewers.
In 1854, Dr John Snow traced a Soho cholera outbreak to the Broad Street water pump. By removing its handle, he showed cholera spread through contaminated water, not "bad air".
Why change was still slow: many MPs believed in laissez-faire and hated spending taxpayers' money or interfering in people's lives — so reform came only when disease became impossible to ignore.

Industrial · government acts
Government finally acts
Under pressure from cholera and reformers, the government moved from doing nothing to taking responsibility:
Public Health Act 1848 — set up a Board of Health and allowed (but did not force) towns to improve water and sewers. Weak, and abolished in 1858.
The "Great Stink" of 1858 — the stench of sewage in the Thames pushed Parliament to fund Joseph Bazalgette's huge London sewer system.
Public Health Act 1875 — a compulsory Act forcing councils to provide clean water, drainage and sewers, and appoint medical officers. A real turning point.
Compare the two Acts: 1848 was voluntary and weak; 1875 was compulsory and effective. The shift shows the state accepting real responsibility for public health.

Modern · c1900–1948
The Liberal reforms and beyond
By 1900 poverty studies by Booth (London) and Rowntree (York) shocked the nation by showing how many people were too poor to stay healthy. The Liberal government (1906–14) acted:
Free school meals (1906) and school medical inspections (1907) for children.
Old Age Pensions (1908) for the elderly.
National Insurance Act (1911) — gave some workers sick pay and medical care.
Why it mattered: for the first time, national government took responsibility for people's welfare, not just sewers — a big step towards the modern welfare state.
Modern · 1948
The birth of the NHS
The Second World War and the Beveridge Report (1942) — which attacked the "five giants" of Want, Disease, Ignorance, Squalor and Idleness — created huge support for change. In 1948, health minister Aneurin (Nye) Bevan launched the National Health Service (NHS).
Healthcare became free at the point of use — funded by taxation, not by whether you could pay.
It covered doctors, hospitals, dentists and glasses for everyone.
The NHS was the greatest single change in the story: the state now guaranteed everyone the right to healthcare.

Modern · today's challenges
New problems, new campaigns
With clean water, sewers and the NHS, infectious diseases like cholera are no longer the main killers. Today's health problems are more about lifestyle:
Smoking, poor diet, obesity and lack of exercise now cause much ill health.
Government uses health campaigns, laws (e.g. the smoking ban in enclosed public places, 2007) and taxes to change behaviour.
Debate continues over how far the state should tell people how to live ("nanny state").
The big change over time: government moved from doing nothing (laissez-faire) to providing water and sewers, then to guaranteeing healthcare, and now to shaping lifestyles.

Government — from laissez-faire to compulsory Acts and the NHS.

Individuals — Chadwick, Snow, Bazalgette, Bevan.
War, crisis & the economy — cholera, the Great Stink and WWII all forced action.
Turning points to argue for: the 1875 Act (government accepts responsibility) and the 1948 NHS (healthcare for all) are the two strongest "biggest change" answers.

You will handle sources — reports, cartoons, photographs, maps — about living conditions. Two habits win marks:
Ask who made it and why (provenance). A reformer's report (like Chadwick's) is designed to shock and persuade; a cartoon may exaggerate to make a point.
Use your own knowledge to judge it. Support or challenge a source with facts you know about the period.
Interpretations differ: historians disagree about what mattered most — was it reformers like Chadwick, government Acts, or scientific ideas like germ theory? Being aware that interpretations vary is exactly what the exam rewards.
